A brake-pad warning light, a squeal or grinding noise when braking, longer stopping distances, or less than about 3 mm of pad material visible through the wheel. Grinding usually means the backing plate is touching the rotor and needs immediate attention.

Check the rotors' thickness against the minimum stamped on them and look for grooves or vibration. Rotors that are worn, scored or below minimum thickness should be replaced with the pads so the new pads bed in on a fresh surface.

On many European cars, yes. The sensor is designed to be worn through by the pad; once the warning has come on, or if the sensor was damaged during removal, it has to be replaced.

Follow the pad maker's bedding procedure, usually several moderate stops from speed without coming to a complete stop, then letting the brakes cool. Avoid hard stops for the first few hundred miles.
